Creating evaluation forms

Create a new form

1. In Eval Studio, choose the tab for the form type you want.
2. Tap New to create a form.
3. Enter a title and complete the required fields.

Skill Sheets are based on a Skill. If you do not see the Skill you need, use Manage Skills & Competencies to add it before creating the form.

Build sections and criteria

1. Add sections to organize the form.
2. Add criteria under each section.
3. Use nested criteria for complex rubrics.

Configure scoring and critical failures

1. Choose a scoring scale.
2. Set critical fail criteria if needed.
3. Review the cut score and overall scoring rules.

Save and generate a PDF

1. Click Save.
2. Open the form actions menu and select PDF to generate a blank PDF.
